Overview
We are seeking a dedicated and skilled Physical Therapist to join our healthcare team. The ideal candidate will possess a strong foundation in patient care and rehabilitation, with experience in various settings including inpatient, outpatient, and home health. Responsibilities
- Conduct comprehensive assessments of patients to develop individualized care plans.
- Implement physiotherapy treatments based on evidence-based practices.
- Collaborate with multidisciplinary teams to ensure cohesive patient care.
- Monitor patient progress and adjust treatment plans as necessary.
- Provide education to patients and their families regarding treatment protocols and home exercises.
- Engage in discharge planning to facilitate smooth transitions from acute care settings.
- Maintain accurate documentation of patient interactions and treatment outcomes using EMR systems.
- Utilize knowledge of anatomy, physiology, and medical terminology in the evaluation and treatment processes.
- Specialize in various areas such as pediatrics, geriatrics, orthopedics, and trigger point therapy as needed.
Qualifications
- Valid state licensure as a Physical Therapist.
- Strong knowledge of physiotherapy techniques and practices.
- Experience in acute care, inpatient, outpatient, or home health settings is preferred.
- Familiarity with discharge planning processes and care plan development.
- Proficiency in EMR systems for documentation purposes.
- Excellent communication skills for effective interaction with patients and healthcare teams.
- Ability to work collaboratively within a multidisciplinary team environment.
