We are currently hiring Supports Planners/Case Managers who reside in the Montgomery County, MD area to work in Maryland’s dynamic Community First Choice program. We seek highly motivated individuals with a passion for advocacy and serving vulnerable populations with complex needs. This role involves helping individuals overcome barriers and connecting them to program and community services, ensuring they can live safely in the community with the necessary supports.
Primary Duties:
- Develop person-centered plans that encompass program and community services.
- Conduct monthly calls and quarterly visits to assess participants’ needs, supports, and service delivery.
- Conduct team meetings and negotiate to resolve conflicts.
- Work with vulnerable populations of all ages with a range of complex needs.
- Accurately input daily billing/documentation in the MD LTSS tracking system.
- Coordinate with local health departments, Maryland Department of Health, community and program providers, and other stakeholders to ensure the delivery of quality and person-centered services.
- Train participants and providers on program eligibility and services, self-direction, and technology systems.
- Maintain accurate, up-to-date records and files for each participant served.
- Identify, document, and communicate all reportable events, including abuse, neglect, and/or exploitation, to appropriate authorities.
- Manage applicants’ enrollment into the CFC, CPAS, HCBOW, or ICS programs.
- Facilitate the transition of participants from nursing facilities to the community.
- Follow all COMAR regulations and company policies.
- Perform other duties as assigned.
Qualification Requirements:
- Bachelor’s or master’s degree in a human services field (e.g., psychology, social work, sociology, nursing, counseling, or a related field).
- At least one year of case management experience and/or experience working with individuals with medical, mental health, and developmental disabilities.
- Experience working with vulnerable populations of various ages, races, ethnicities, and cultures.
- Experience with Medicaid-funded programs.
- Strong critical thinking and conflict resolution skills.
- Excellent written and oral communication skills.
- Experience writing person-centered plans and locating community resources.
- Highly organized, with an understanding of participant choice and empowerment.
- Ability to work from the main office during the initial training period and attend all company and state in-person and virtual trainings and meetings.
- Designated home office space that meets all HIPAA requirements, with a high-speed internet connection.
- Strong computer skills and familiarity with Microsoft Office Suite.
- Ability to type at least 35 WPM.
Transportation Requirements:
- Ability to travel locally within 40 miles and/or 45 minutes from your current residence.
- Reliable transportation for weekly visits, with a valid driver’s license and a good driving record.
