About the Role
As a Case Manager - Skilled Nursing, you will be responsible for coordinating and managing the care of our residents, ensuring that their physical, emotional, and social needs are met. You will work closely with our interdisciplinary team, including nurses, therapists, and social workers, to develop and implement individualized care plans that promote resident independence, dignity, and well-being.
Responsibilities
Conduct initial and ongoing assessments of residents' needs and develop individualized care plans
Coordinate care with healthcare providers, family members, and caregivers to ensure seamless transitions and optimal outcomes
Monitor and evaluate the effectiveness of care plans, making adjustments as needed to ensure resident satisfaction and achievement of goals
Maintain accurate and comprehensive records of resident care and progress
Identify and report any changes in resident condition or status to the healthcare team
Collaborate with interdisciplinary team to resolve conflicts and address concerns
Provide emotional support and counseling to residents and their families
Participate in quality improvement initiatives to enhance resident care and satisfaction
Requirements
Bachelor's degree in a health-related field (e.g., nursing, social work, occupational therapy)
Minimum 2 years of experience in case management or a related field
Strong communication and interpersonal skills
Ability to work effectively in a fast-paced, dynamic environment
Familiarity with Medicaid and Medicare regulations and guidelines
