Job Summary
We are seeking a compassionate and skilled Registered Nurse (RN) with experience in tracheostomy and ventilator care to provide high-quality, patient-centered nursing services in the home setting. The RN will be responsible for delivering skilled nursing care, performing clinical assessments, coordinating care with caregivers and providers, and ensuring patient safety and comfort. Experience with medically complex clients is required
Duties
- Conduct comprehensive patient assessments including vital signs, physical examinations, and physiological evaluations.
- Administer medications, injections, IV infusions, and perform phlebotomy with adherence to aseptic techniques and HIPAA regulations.
- Manage airway techniques, ventilator support, and perform advanced procedures such as tube feeding and gastric lavage as needed.
- Document all patient interactions accurately using EMR/EHR systems such as Epic, Cerner, Athenahealth, or eClinicalWorks.
- Coordinate discharge planning and case management for diverse populations including geriatrics, pediatrics, neonatal patients, and individuals with developmental disabilities or dementia.
- Provide specialized care in areas such as cardiac catheterization, trauma medicine (Level I & II Trauma Centers), infectious disease care, hospice & palliative medicine, and post-acute settings.
- Participate in diagnostic evaluations including laboratory specimen collection/processing and medical imaging procedures like sonography or mammography.
- Support infection control practices and maintain sterile processing standards across all clinical activities.
- Educate patients on health coaching topics such as nutrition, family planning, medication management, and post-discharge instructions.
- Collaborate with multidisciplinary teams to ensure compliance with NCQA standards and ICD coding requirements for accurate medical documentation.
Skills
- Proven experience in various clinical settings including hospital medicine, ICU (including PICU), emergency room/urgent care centers, outpatient clinics, or home & community care environments.
- Proficiency in EMR/EHR systems such as Epic, Cerner, Athenahealth or eClinicalWorks; familiarity with ICD-9/ICD-10 coding is preferred.
- Strong knowledge of medical terminology, anatomy & physiology, diagnostic evaluation techniques, and pharmacology for medication administration.
- Skilled in performing advanced procedures like suturing, catheterization (urinary & vascular), ostomy care, spinal tap procedures, and neonatal ventilator management.
- Ability to work effectively with diverse populations including children (toddler & neonatal care), seniors (geriatrics & memory care), individuals with disabilities or developmental challenges.
- Excellent critical thinking skills for triage assessment and acute pain management; capable of working under pressure in trauma or emergency situations.
- Knowledge of infection control protocols and sterile technique to prevent healthcare-associated infections.
- Strong communication skills for patient education and interdisciplinary collaboration; commitment to maintaining HIPAA compliance at all times. This position requires a current RN license with relevant clinical experience across multiple specialties. Candidates should demonstrate flexibility to adapt to various healthcare environments while maintaining the highest standards of patient safety and quality care.
Pay: $45.00 - $50.00 per hour
