As a full-time field LVN, you will provide skilled nursing care in the home setting while collaborating closely with the Director of Nursing and the clinical team. This is a hands-on role for nurses who value professionalism, strong documentation, and patient-centered care.
What You’ll DoKey Responsibilities
- Deliver skilled nursing visits in accordance with the plan of care
- Complete accurate, timely clinical documentation in the EMR
- Communicate clearly with patients, families, physicians, and the clinical team
- Coordinate with scheduling/operations to ensure timely visits and continuity of care
- Maintain compliance with agency policies and home health regulations
- Participate in case conferences and support quality improvement efforts (as assigned)
Compensation & Pay DetailsBase Pay
- $30.00 per hour (non-exempt/hourly)
- Overtime: 1.5x after 40 hours in a workweek ($45/hr)
- Pay frequency: Bi-weekly
Note: All compensable time—including visits, drive time between visits, and documentation—must be recorded daily.
Productivity Expectations & Monthly Bonus
HHCN uses a productivity points system to support balanced workloads and consistent patient care.
- Minimum weekly expectation: 35 points (average 7/day)
Monthly bonus (based on weekly points achieved)
On-Call Rotation (Condition of Employment)
On-call participation is required for full-time field nursing.
- Weeknights: Mon–Fri 5:00 PM – 8:30 AM
- Weekends: Fri 5:00 PM – Mon 8:30 AM
- Standby stipend: $125 per assigned week
- Call response premium: +$10/hr above base rate for hours actively worked

What We’re Looking For (Qualifications)Required
- Active Texas LVN license in good standing
- Ability to complete documentation accurately and on time
- Strong communication, reliability, and professionalism
- Comfortable working independently in the field while staying connected to the team
- Must meet onboarding requirements (background check, MVR check, and I-9 verification)
- Prior home health experience
- Familiarity with common home health EMR workflows
