*Client Care Plan*
We are looking to hire for two different weekend full time positions. These shifts are both 9A-9P every Saturday and Sunday. We are looking for someone dependable who has a passion for helping others. Both of these shifts are for part time hours but there is an opportunity for full time hours if you are willing to work another client for a few days Monday-Friday. When we speak over the phone, we will go over the care plans for both clients to see which client you would be a better fit for. We look forward to speaking with you!

Job Summary
We are seeking a dedicated and experienced Caregiver to join our team for weekend shifts from 9 AM to 9 PM. The ideal candidate will possess a strong background in providing compassionate patient care and supporting individuals with diverse needs. This role offers an opportunity to make a meaningful difference in the lives of clients by assisting with daily activities, ensuring safety, and promoting independence within their homes or community settings.
Responsibilities
- Provide personal care assistance including bathing, grooming, dressing, and mobility support to clients with various needs.
- Monitor patients’ health status by observing vital signs and reporting any concerns to healthcare professionals promptly.
- Assist with activities of daily living (ADLs) such as meal preparation, light housekeeping, and medication reminders.
- Support clients with mobility using appropriate transfer methods and equipment like Hoyer lifts when necessary.
- Ensure a safe environment by adhering to manual handling protocols and maintaining cleanliness standards.
- Communicate effectively with clients, family members, and healthcare teams to coordinate care plans and updates.
- Maintain detailed documentation of patient observations, care provided, and any incidents in compliance with HIPAA regulations.
Requirements
- Proven experience working as a caregiver in group homes, assisted living facilities, nursing homes, or home health settings.
- Knowledge of patient observation techniques, medication administration, and vital signs monitoring.
- Familiarity with dementia care, Alzheimer’s care, memory care, and supporting individuals with neurodevelopmental or developmental disabilities.
- Ability to perform manual handling tasks safely and effectively, including patient transfers using Hoyer lifts or other equipment.
- Certification in CPR and First Aid additional training in hospice or hospice care is a plus.
- Strong communication skills with the ability to engage empathetically with clients of all ages and backgrounds.
- Experience working within HIPAA guidelines ensuring confidentiality and privacy of patient information.
- Physical stamina for heavy lifting and long periods of standing or assisting clients with mobility needs.
