Licensing & Certification Requirements: Hold a current, unencumbered license in Louisiana as a Licensed Practical Nurse or Registered Nurse; must be registered with the Louisiana State Board of Examiners of Practical Nurse Examiners or the RN board as appropriate.
Education Requirement: Must be a graduate of an accredited educational institution (school of nursing) or a graduate of a state approved school of practical nursing.
Experience: At least two (2) years of nursing experience preferred, with one (1) year of wound care experience preferred.
Special Demands: Must be cooperative, willing to work, and courteous to residents, residents' families, and other staff members.
Job Knowledge: Nursing policies & procedures; administration of medication; federal and state laws and regulations relative to resident care; operate within the scope of Nursing Practice Act; facility policies.
Duties & Responsibilities:
1. Perform all wound treatments daily for decubitus ulcers, stasis ulcers, surgical wounds, etc.
2. Document wound appearance, healing, deterioration, etc. in nurses’ notes.
3. Notify weekend supervisor, physician and family representative of new wounds or significant change in wound status with accompanying documentation.
4. Notify weekend supervisor if any supplies is needed for wound treatments.
5. Complete full body skin assessments on all admits and readmits that occur over the weekend.
6. Complete QA checklist on all new wounds. Follow-up with physician/DON on any areas which need addressing.
7. Focus on measures to prevent wounds when making rounds or treating a wound (turn schedules posted and followed, moisture barrier used on incontinent residents, heel protectors, heel floated, pressure reducing mattresses, etc.)
8. Attend In-service Meetings to keep current on wound guidelines and treatment.
9. Other reasonable job duties as assigned by DON or Administrator
