Starting Annual Salary: $49,000 to $50,004 equivalent from $ 25.13 to $25.64 based on a 37.5 hours weekly work schedule
POSITION SUMMARY
The Supportive Services Case Manager will coordinate services and provides case management and referrals for residents of Kalmia Rose at Jordan Downs as contracted by Bridge Housing.
JOB DUTIES
- Provides case management, outreach services, and service coordination to Residents (parents and youth).
- Builds a foundation of specific community resources that include a bank of resources with contact information and information on their referral processes.
- Assists Residents to locate community resources, especially supports that will help them develop self-sufficiency and maintain housing at Kalmia Rose.
- Oversees specific activities provided to residents such as community groups/meetings.
- Links resources to residents
- Coordinates events such as resource/Health and Job fairs for residents.
- Works with identified El Nido, BRIDGE Housing, Choice Neighborhood initiative, and HACLA
- Maintain resident daily attendance log; and input attendance into Salesforce.
- Tracks work with residents for El Nido records.
- Collaborates with BRIDGE Housing Property Management to ensure support for residents.
- Works in collaboration with Bridge Housing to develop service delivery model and collect survey data.
- Represent El Nido at Community Meetings.
- Attend training as needed.
- Assist in the overall program goals and services to successfully implement program.
POSITION REQUIREMENTS
- Bachelor’s degree with a minimum of two years of similar social services experience or non-degree with a minimum of 3 years of experience.
- Three to four years relevant case management experience preferred.
- Proficient in MS word and Excel; accurate, math skills, ability to connect with residents and community members.
- Bilingual/Spanish proficiency
- Ability to work some weekends and evenings.
- Training/Experience in community organizing and resources.
- Ability to lift 30lbs, climb stairs and walk- to outreach to residents/clients
- Travel to work between sites
- Possession of a car, valid driver's license, satisfactory driving record and proof of insurance
