The Homestead Home Health and Hospice is seeking a dedicated and compassionate Registered Nurse (RN) Case Manager to join our team in Rexburg, Idaho. This is a rewarding opportunity to provide personalized, high-quality care to patients in the comfort of their homes in the community and at The Homestead Assisted Living Facility.
Position Details:
- Employment Type: Full-time
- Schedule: Monday–Friday 9am-5pm (with occasional weeknight and weekend on-call responsibilities)
- Location: Rexburg, ID
- Pay: Starting at $30/hr or more based on experience; our nurses are paid by the hour (not by the visit) with guaranteed 40 hours per week!!
Key Responsibilities:
- Manage and coordinate patient care for home health and hospice patients
- Perform comprehensive patient assessments and develop individualized care plans
- Collaborate with physicians, interdisciplinary team members, patients, and families
- Provide skilled nursing care and patient/family education
- Ensure accurate and timely documentation in company’s EMR system and in compliance with federal regulations
- Participate in on-call rotation
Qualifications:
- Current Registered Nurse (RN) license in Idaho
- Preferred but not required 1–2 years nursing experience (home health or hospice experience is even better!)
- Strong clinical, organizational, and communication skills
- Ability to work independently and manage a caseload effectively
- Compassionate approach to patient-centered care
- Valid driver’s license and reliable transportation
