Licensed Clinical Social Worker (LCSW) – PRN/Per Diem Cherokee County, GA & Surrounding Areas Join Our Growing Team! Piedmont Hospice & Palliative Care is seeking a compassionate and dedicated Licensed Clinical Social Worker (LCSW) to join our team on a PRN/Per Diem basis, providing care to patients in Cherokee County and surrounding communities. Responsibilities
- Complete psychosocial assessments for hospice patients.
- Develop and implement individualized plans of care. Provide emotional support, counseling, and crisis intervention to patients and families.
- Assist with advance care planning and community resource coordination.
- Collaborate with physicians, nurses, chaplains, and the interdisciplinary care team.
- Maintain accurate, timely, and compliant documentation.
- Participate in interdisciplinary team meetings as appropriate.
Experience
- Current and unrestricted Georgia Licensed Clinical Social Worker (LCSW) license.
- Minimum 1 year of experience preferred. Hospice, palliative care, home health, or healthcare experience preferred. Excellent communication and interpersonal skills.
- Strong organizational and documentation abilities. Current and valid CPR certification or willingness to obtain certification before hire.
- Current driver's license, reliable transportation, and proof of automobile insurance.
- Ability to travel throughout Cherokee County and surrounding areas.

Pay: $65.00 - $75.00 per hour
