We are seeking a compassionate and skilled Physical Therapist (PT) to join a Home Health and Hospice team serving patients across the East Bay Area. This role offers meaningful one-on-one patient care with a consistent weekday schedule.

Role Overview As a Home Health Physical Therapist, you will provide individualized therapy in patients’ homes, helping them improve mobility, manage pain, and enhance overall quality of life. You’ll collaborate closely with a multidisciplinary care team to deliver patient-centered care.
Responsibilities
- Conduct in-home physical therapy evaluations and assessments
- Develop and implement individualized treatment plans
- Educate patients and caregivers on exercises and safe mobility techniques
- Monitor and document patient progress per agency and regulatory guidelines
- Coordinate care with nurses, occupational therapists, physicians, and other team members
- Provide clinical updates and adjust care plans as needed
- Ensure compliance with all state, federal, and agency policies
