Job Description Summary
As a Home Health RN, you will play a vital role in delivering compassionate, skilled nursing care to patients in the comfort of their homes. You will work closely with patients, families, caregivers, and physicians to ensure high-quality, patient-centered care in compliance with California regulations and agency policies.
Key Responsibilities:
- Provide professional nursing care as prescribed by the physician and in accordance with state, federal, and agency standards.
- Conduct comprehensive initial assessments and ongoing evaluations of patient status and care needs.
- Develop, implement, and adjust individualized care plans based on patient condition and progress.
- Administer medications and treatments safely and accurately, monitoring for effectiveness and side effects.
- Deliver high-quality respite shift care for vulnerable patients, which may include:
- Tracheostomy management and suctioning
- Ventilator monitoring and support
- G-tube feedings and medication administration
- Ongoing assessment and emergency response as needed
- Educate patients and their families/caregivers on disease management, medication use, self-care techniques, and safety practices.
- Collaborate with the interdisciplinary team to review medical necessity, coordinate services, and support discharge planning.
- Maintain accurate, timely, and thorough documentation of all patient care activities.
- Promote and maintain a safe environment for patients in the home setting.
- Serve as a resource and provide hands-on supervision when needed to ensure quality and continuity of care.
