Assist patients with daily personal care needs in a compassionate and respectful manner.

Support family members in caring for their loved ones.

Observe and report changes in patient conditions to the care team.

Maintain accurate and timely documentation of services provided.

Collaborate with nurses, physicians, and other hospice professionals to ensure high-quality patient care.

High school diploma or GED required.

Current Ohio Certified Nursing Assistant (CNA) credential preferred.

At least one year of related healthcare experience preferred.

Certification in Hospice and Palliative Nursing Assistance (CHPNA) is preferred. Ohio’s Hospice provides support and resources for employees interested in obtaining certification.

Basic computer proficiency for electronic documentation.

Ability to travel and drive safely in varying weather and road conditions.

Valid driver's license and acceptable driving record.

Proof of automobile insurance meeting state requirements.
-
Successful completion of a criminal background check.

Ability to pass a 10-panel drug screening.
