We’re hiring in numerous programs and across different territories to include San Francisco, Alameda, San Mateo, San Jose, and Sonoma
Current openings include Outreach roles, clinic and community-based Care Managers, and Clinical Supervisors.
Programs Hiring social workers:
- CalAIM ECM – Community-based case management supporting older adults with complex medical and social needs.
- Acute Care – Hospital-based social work supporting discharge planning and transitions of care.
- Justice Involved - Supports those transitioning from correctional settings into the community by connecting them to medical, behavioral health, housing, and social services.
- PACE – Clinic-based interdisciplinary care supporting older adults through our all- inclusive On-Lok PACE innovative model of care.
Open Roles:
Care Manager I (CMI)
- Bachelor’s in Social Work or related field
- $72K–$84K
Care Manager II (CMII)
- MSW or related Bachelor’s degree plus 2 additional years of experience
- $85K–$102K
Care Manager III (CMIII)
- MSW or related clinical Master’s degree (ex: MFT); LCSW/LMFT preferred or RN background
- $95K–$113K
Clinical Supervisor
- Masters’ degree in Social Work (M.S.W.) or within a related field
- $117-$125k
