Job Overview:
We are seeking a dedicated Per Diem Registered Nurse to join Jersey Family Home Health Care. You will be responsible for opening cases, conducting in-home assessments, and supporting caregivers (CHHAs) while ensuring compliance with care standards.
Key Responsibilities:
· Conduct in-home assessments and reassessments.
· Develop and implement individualized plans of care.
· Orient and assess the skills of Certified Home Health Aides (CHHAs).
· Perform CHHA performance reviews and provide support.
· Complete CAHC-required Documentation (Accreditation).
· Collaborate with the client service team to ensure high-quality care.
Requirements:
· Active RN license in State.
· Experience in-home health care preferred.
· Strong organizational and communication skills.
· Proficiency in completing CAHC-related paperwork is a plus.
Compensation & Benefits:
· $70 for Initial Assessment (Must be physically at the client’s residence)
· $60 for Case Monitoring (Every 60 days, must be physically at the client’s residence)
· $.65/mile reimbursement
· $45/hr. to revise Clients and Certified Home Health Aides folders in the office
· $250 sign on bonus after 6 months of employment
· Flexible per diem schedule.
