Overview
We are seeking compassionate and dedicated Certified Home Health Aides (CHHA) to join our team in. One position is for a client in Medford and the other is for a live-in client in Hamilton (Atlantic County). As a vital part of our caregiving team, you will deliver personalized assistance, promote independence, and ensure the safety and well-being of our clients. Your energetic approach and commitment to excellence will make a meaningful difference in the lives of those you serve.
Responsibilities
- Provide patient personal care assistance, including assisting with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, and mobility support
- Monitor patient health by observing vital signs and reporting changes promptly using Electronic Health Record (EHR) systems or Medical Records documentation
- Administer medications accurately following home healthcare regulations and medical terminology guidelines
- Assist with specialized care needs such as Alzheimer's, dementia, memory care, hospice, and end-of-life support
- Perform patient transfers using safe methods like Hoyer lifts or other transfer techniques while maintaining patient safety and comfort
- Maintain patient confidentiality in accordance with HIPAA regulations and ensure proper documentation of medical records and care activities
- Support clients with light housekeeping tasks including cleaning, laundry, meal preparation, and ensuring a safe environment
Experience
- Prior experience working with individuals with developmental disabilities, geriatrics, or in assisted living or nursing home settings is highly preferred
- Experience working with EMR/EHR systems for accurate medical documentation and patient record keeping
- Proven ability to work independently in a home health or community care setting while maintaining high standards of patient care
- Familiarity with home health agency operations, home & community care regulations, and hospice or palliative care practices is advantageous
- Strong communication skills for effective client interaction and collaboration with healthcare teams
- Ability to perform heavy lifting when necessary and handle physically demanding tasks safely
