Compensation: $21.00 per hour. $1.00 per hour shift differential on weekends.

Assists patient with all aspects of personal and oral hygiene which includes bathing (complete and assisted), care of hair and skin and backrubs.

Accurately takes and records vital signs.

Safely assists patients with transfers to bed, chair and commode.

Safely assists bed-bound patients with appropriate range of motion exercises and required turning and positioning.

Performs accurate recording of intake and output (I&O).

Follows a bowel/bladder training program; provides bowel/bladder incontinence care and assists with all aspects of toileting.

Performs all aspects of catheter care according to policy and procedure.

Applies colostomy bags, anti-embolic stockings, braces and simple dressings as instructed and demonstrated by the RN.

Safely administers tepid sitz baths; assists patients in self-administering prescribed medication.

Demonstrates safe and effective use of hoyer lift as instructed and demonstrated by the RN.

Prepares patient's meals and assists with feeding according to dietary instructions.

Observes the patients physical and emotional status and reports on a timely basis any changes in the patient’s condition or family situation.

Assists with post-mortem care as instructed by the RN.

Accurately completes all aspects of documentation and submits via Celltrack.

Safely changes occupied or unoccupied bed.

Keeps patient environment neat and clean.

Incorporates consideration of the physiological and developmental age of patients into the overall plan for the delivery of patient care.

Participates in all required in service education programs and meetings.

Demonstrates a commitment to own work schedule by adhering to attendance standards. Provides proper notification in advance for absence and tardiness.

Adheres to employment policies in accordance with the Employee Handbook

Demonstrates compliance with the dress code policy by appearing well groomed, wearing ID badge while on duty and maintaining a professional appearance.

Requirements:
Required Skills/Abilities:

Knowledge of skills required to provide excellent care in accordance with the role of Certified Home Health Aide (CHHA) in an in-home hospice environment.

Self-starter who can work independently under the direction of the Registered Nurse.

Excellent interpersonal skills for interacting with patients and families, nurses and employees, and patients and families via telephone and in person.

Ability to comprehend the spoken and written English language, to communicate with clarity both verbally and in writing.

Well organized to manage time, prepare care-related documentation.

Ability to utilize a cell phone and GPS to move about as assigned to various patient homes throughout the service area.
Education and Experience:
Required Education:

High School diploma or equivalent.

Must possess current NJ Board of Nursing Home Health Aide Certification.
