Duties Include:
The Caregiver will follow a plan of care and provide daily assistance to patients. This includes:
- Following an established Care Plan and providing personal care services for all patients
- Including: bathing, hygene, laundry, cleaning, whatever other services are prescribed by superivising clinician
- Completing documentation for all visits including time sheets
- Communicating any requests or changes for patient care
- Coordinating with agency staff to ensure care is being performed as needed

Job Requirement:
- Certified Home Health Aide license to work in CA
- Minimum 1 year of Home Health experience
- CPR / ALS Card
- Driver’s License or ID Card
- Current Negative TB Test
- Current Physical Exam
- COVID Vaccine Card or Exemption Proof
- Able to do Home Health Visits in San Pablo, Richmond and nearby cities
