Heritage Grove Estates is seeking a compassionate, dependable, and clinically skilled Licensed Practical Nurse (LPN) to join our Night Shift nursing team.
We are offering a $7,000 Sign-On Bonus for eligible Full-Time Night Nurses, in addition to a Night Shift Differential.
As a Night Shift LPN, you will provide direct nursing care, administer medications and treatments, monitor changes in resident condition, supervise and support CNAs, and help ensure residents receive safe, timely, and individualized care throughout the overnight shift.
Our LPNs are trusted members of the clinical team and play an essential role in maintaining continuity of care. You will work closely with RNs, physicians, nursing leadership, CNAs, and other interdisciplinary team members while exercising clinical judgment within your scope of practice.
This position is ideal for a nurse who values hands-on resident care, professional accountability, teamwork, and the opportunity to make a meaningful difference during every shift.
Provide direct nursing care in accordance with individualized care plans, physician orders, professional standards, and facility policies
Monitor residents throughout the night and promptly recognize changes in physical, cognitive, behavioral, or psychosocial condition
Report significant changes in condition to the appropriate RN, physician, nursing leadership, and responsible party in accordance with facility protocols
Administer medications, treatments, and therapeutic interventions safely and accurately within the LPN scope of practice
Complete scheduled treatments and ensure resident clinical needs are addressed throughout the assigned shift
Obtain and monitor vital signs, laboratory results, treatment outcomes, and other clinical information as appropriate
Respond promptly to resident needs, urgent situations, and changes in condition using sound nursing judgment
Assist with admissions, readmissions, transfers, and discharges as assigned
Communicate effectively during shift-to-shift report to promote continuity and accuracy of resident care
Collaborate with RNs, physicians, pharmacy, therapy, dietary, social services, and other interdisciplinary team members to coordinate resident care
Provide direction, delegation, and support to CNAs and other nursing support personnel during the assigned shift
Ensure resident assignments are completed appropriately and in accordance with individualized care plans
Monitor the quality and timeliness of care provided by nursing assistants
Provide real-time coaching, education, and clinical guidance to nursing support staff
Promote accountability, teamwork, respectful communication, and professional conduct throughout the shift
Report staffing, performance, resident care, or safety concerns to nursing leadership promptly
Assist with maintaining appropriate floor coverage and resident care during changing staffing needs
Maintain accurate, complete, and timely documentation within the electronic medical record (EMR)
Document medication administration, treatments, assessments, changes in condition, physician notifications, and resident responses appropriately
Ensure physician orders and clinical recommendations are implemented and followed through as assigned
Maintain compliance with applicable CMS requirements, Kansas regulations, facility policies, and professional nursing standards
Protect resident privacy and confidentiality in accordance with HIPAA and facility requirements
Participate in facility survey readiness and maintain compliant nursing practices throughout each shift
Follow infection prevention practices, including hand hygiene, appropriate PPE use, isolation precautions, and transmission-based precautions
Participate in quality assurance and performance improvement initiatives
Monitor for and promptly report resident safety concerns, falls, skin concerns, medication issues, infections, changes in condition, and other clinical risks
Follow established procedures for incidents, accidents, allegations, and other reportable events
Promote a safe, clean, respectful, and resident-centered clinical environment
Support facility initiatives designed to improve resident outcomes and reduce preventable adverse events
Graduate of an accredited practical nursing program
Current, active Licensed Practical Nurse (LPN) license in good standing in the State of Kansas
Current CPR or Basic Life Support (BLS) certification
Skilled nursing, long-term care, rehabilitation, or post-acute nursing experience preferred
Strong clinical observation, assessment, and critical-thinking abilities within the LPN scope of practice
Ability to recognize changes in resident condition and escalate concerns appropriately
Demonstrated ability to delegate, supervise, and support nursing assistants
Excellent communication and interpersonal skills
Strong organizational and time-management abilities
Ability to prioritize multiple resident needs in a fast-paced clinical environment
Dependable attendance and ability to work effectively as part of the night-shift clinical team
