Job Summary
Care Manager will connect with patients of PHSC, by phone, or electronically. The primary focus of the role will be to work closely with our patients to support them in becoming active in their health care, better understanding their illness and coordinating their care. Care Manager will work closely with Medicaid Managed Care Companies, Medicare Plans, and Private Insurance Plans to ensure that patient gaps in care are closed, and patient outcomes improved. Care Manager will also work closely with Providers, Nursing Support Staff, and Front Desk and Intake Staff to ensure that patients are scheduled at appropriate intervals according to their diagnosis and identified gaps in care.
Qualifications
Required
1. Three to five years of nursing/CMA experience, preferably in federally qualified health centers, home health, hospitals, ambulatory care, community public health, case management, or care coordination across multiple settings and with multiple providers.
2. Current unencumbered Louisiana Registered Nurse license, Louisiana Practical Nurse license or Certified Medical Assistant certification.
Preferred
1. Familiarity with the community we are serving or commitment to learn and understand through on the ground networking, community assessment, etc.
2. Knowledge and experience activating patients and teaching self-management skills.
3. Experience working with vulnerable populations (geriatrics, minorities, behavioral health).
4. Ability to navigate ambiguity with the aid of structured problem solving techniques.
5. Exceptional communication skills, both written and oral, ability to positively influence others with respect and compassion.
6. Strong work ethic built on a foundation of proactivity, collaboration, and teamwork.
7. Committed to the practice of inquiry and listening.

Responsibilities
1. Conduct comprehensive assessments that include the medical, behavioral, pharmaceutical and social needs of the patient, identify gaps in care and barriers to attaining improved health.
2. Based on this assessment, and in conjunction with the patient, the patient’s provider, third-party payers, and other members of the care team, create and implement a care plan that will address the identified needs, remove the barriers and improve the health of the patient.
3. Coordinate care by serving as the advocate and resource for the patient, their family and their physician, building effective relationships in the community and across the continuum of care.
4. Assess the patient’s knowledge of their clinical condition and provide education and self-management support based on the patient’s unique learning style.
5. Coordinate care across settings and with other providers, including the safe management of care transitions. Care Manager will monitor patients for frequent emergency room usage, will monitor inpatient hospital discharge summaries, and monitor the referral process to ensure that PHSC patients receive the appropriate care from outside specialists and are brought back to PHSC for continued primary care.
6. Provide clinical oversight to non-licensed support staff (e.g. health coaches, patient navigators, community health specialists, etc.) and delegate supportive tasks as appropriate.
7. Work with the patient and their caregiver to increase their self-efficacy and ability to play a central role in their care.
8. Measure, improve and maintain quality outcomes (clinical, financial, and functional) for individual patients and the population served.
9. Comply with applicable law and fully compliant and cooperate with PHSC’s Compliance Program.
10. Any other assignments deemed necessary.
