Job Summary
We are seeking a compassionate and dedicated Licensed Clinical Social Worker (LCSW) to join our team. The ideal candidate will possess a strong background in mental health counseling and demonstrate expertise in various therapeutic modalities. This role involves providing high-quality psychotherapy, crisis intervention, and case management services to individuals and families, particularly focusing on children and those with developmental disabilities. The LCSW will work collaboratively with other healthcare professionals to ensure comprehensive patient care.
Responsibilities
- Conduct thorough patient assessments and diagnostic evaluations to determine mental health needs.
- Provide individual counseling sessions utilizing evidence-based practices such as cognitive behavioral therapy, psychodynamic therapy, and motivational interviewing.
- Develop and implement treatment plans tailored to meet the unique needs of patients, including those with PTSD, addiction issues, or behavioral challenges.
- Engage in crisis management and intervention strategies to support patients in acute distress.
- Facilitate coordination of services to enhance communication and support systems within the home environment.
- Maintain accurate medical documentation in compliance with HIPAA regulations, including EMR systems for medical records management.
- Collaborate with interdisciplinary teams to coordinate care for patients, including those requiring home and community care services.
- Provide education and support regarding mental health issues to patients and their families, fostering a deeper understanding of available resources.
Qualifications
- Master’s degree in Social Work from an accredited institution; current LCSW licensure required.
- Proven experience in psychotherapy, crisis intervention, case management, and behavioral health practices.
- Experience working with geriatrics is highly desirable.
- Excellent interpersonal skills with the ability to build rapport with diverse populations while maintaining professional boundaries.
