At Greater Love and Care, we believe in providing personalized support to our clients. A critical component of this care is the initial client assessment. As an RN on our team, you will play a vital role in gathering essential information to develop individualized care plans to ensure the highest quality of care for our clients.
Responsibilities
- Perform comprehensive physical, psychosocial, and functional assessments for patients in their homes, hospitals, or rehab centers.
- Document client conditions, and needs, using this data to inform and adapt individualized care plans.
- Collect and analyze clients' medical history, current symptoms, medication lists, and social determinants of health.
- Ensure clients' safety by assessing home environments and identifying potential risks.
- Provide compassionate support to the client and families.
- Assist in critical care situations, demonstrating the ability to respond quickly and effectively if needed.
- Maintain accurate and confidential client records in accordance to HIPPA regulations and company policy.
- Adhere to all nursing standards of practice, ethical guidelines, and state/federal regulations.
Requirements:
Candidates should have a valid Registered Nurse license in good standing and demonstrate proficiency in the following areas:
- Excellent communication and interpersonal abilities.
- Home health or community nursing experience (a plus)
- Geriatric nursing experience (a plus)
- Ability to assess vital signs accurately and record them appropriately.
- Proven experience in conducting comprehensive client assessments.
- Ability to work independently and manage time effectively.
- Must have a valid driver's license and reliable transportation as travel to patient locations is required.
This is a 1099 contractor position, and this role includes mileage reimbursement for travel.
