Job Summary
The Director of Clinical Services - RN Home Health is a leadership role responsible for overseeing the delivery of high-quality home health care services. This position involves managing clinical operations, supervising nursing staff, ensuring compliance with healthcare regulations, and maintaining excellent patient outcomes. The ideal candidate will possess extensive nursing experience, strong management skills, and a deep understanding of home health and hospice care environments. This role requires strategic planning, staff development, and operational oversight to ensure the seamless provision of patient-centered care within the agency’s standards and regulatory requirements.
Responsibilities
- Lead and coordinate all clinical operations within the home health agency, ensuring compliance with Medicare, HIPAA, and other regulatory standards.
- Supervise nursing staff, including nurse managers and direct care nurses, providing mentorship and performance evaluations.
- Develop, review, and update individualized care plans for residents, ensuring they meet medical and psychosocial needs.
- Oversee documentation review, medical records management, and EMR systems to ensure accuracy and confidentiality.
- Monitor patient care quality through case management reviews, patient monitoring data, and documentation audits.
- Manage staffing schedules, shift management, and resource allocation to optimize clinical coverage and efficiency.
- Collaborate with interdisciplinary teams to coordinate services across assisted living facilities, hospice programs, and other healthcare settings.
- Ensure staff adherence to safety protocols related to physiology knowledge, anatomy knowledge, medical terminology, and resident safety procedures.
- Oversee budgeting processes related to clinical services while maintaining cost-effective practices without compromising quality of care.
- Facilitate ongoing staff training on documentation standards, medical records management, Medicare regulations, and EMR systems usage.
- Maintain effective communication with residents’ families regarding care plans and clinical updates involving residents’ <PERSON>.
Experience
- Proven leadership experience in nurse management within home health, hospice care, assisted living, or acute care settings.
- Supervising experience with a strong background in managing multidisciplinary teams in healthcare environments.
- Extensive clinical background with critical care experience preferred; familiarity with physiology and anatomy knowledge essential.
- Demonstrated proficiency in EMR systems and medical records management aligned with HIPAA compliance standards.
- Experience in case management including developing care plans tailored to individual resident needs.
- Knowledge of Medicare regulations related to home health services is highly desirable.
- Strong understanding of medical terminology, documentation review processes, patient monitoring techniques, and shift management strategies.
