Job Summary
We are seeking a dedicated and compassionate Adult Home Health Physical Therapist to provide exceptional patient care in the comfort of patients' homes and community settings. The ideal candidate will have a strong background in physiotherapy, with experience in inpatient, outpatient, hospital, and clinic environments. This role involves developing personalized care plans, managing discharge planning, and working with diverse patient populations including pediatrics and geriatrics. The therapist will utilize their expertise in anatomy, physiology, and medical terminology to improve patient outcomes through tailored therapeutic interventions.
Duties
- Conduct comprehensive assessments of patients’ physical conditions to develop individualized care plans.
- Implement physiotherapy techniques such as trigger point therapy, orthopedics, and geriatrics-focused treatments to promote mobility and pain relief.
- Manage discharge planning by coordinating with healthcare teams to ensure smooth transitions from hospital or inpatient settings to home or community care.
- Deliver patient care across various environments including home health, outpatient clinics, hospitals, and community centers.
- Document patient progress accurately using EMR systems and communicate effectively with other healthcare professionals.
- Educate patients and their families on exercises, injury prevention, and health maintenance strategies.
- Monitor treatment effectiveness and adjust care plans accordingly to optimize recovery outcomes.
- Ensure compliance with healthcare regulations and maintain a safe environment for patients during therapy sessions.
Skills
- Strong knowledge of anatomy, physiology, and medical terminology relevant to physiotherapy practice.
- Experience in discharge planning, acute care, inpatient, outpatient, hospital, home health, and community care settings.
- Proficiency with EMR systems for documentation and patient record management.
- Skilled in pediatrics, geriatrics, orthopedics, and trigger point therapy.
- Ability to develop comprehensive care plans tailored to individual patient needs across diverse populations.
- Excellent patient care skills with a compassionate approach to therapy delivery.
- Familiarity with home & community care protocols and outpatient clinic procedures.
- Effective communication skills for interdisciplinary collaboration and patient education.
- Knowledge of medical procedures related to physiotherapy including orthopedic rehabilitation and geriatrics management.
