A Long-term care facility in Walker County, GA is looking for a Director of Social Services to join our team. We are seeking a passionate and dedicated Social Worker with the desire to make a difference in the lives of our residents, the residents’ family members, and our team members.
Responsibilities:
- Assist in the planning, organization, implementation, and evaluation of the facility’s Social Services Department
- Ensure social services are delivered in compliance with federal, state, and local guidelines and in alignment with the facility’s values and policies
- Help develop and monitor individualized care plans for residents, addressing their social and emotional needs
- Provide ongoing support to residents and families during admissions, transitions, and care conferences
- Recommend appropriate equipment, services, or referrals for outpatient and home health support
- Participate in policy development and recommend improvements to strengthen care delivery
- Organize and participate in in-service training sessions for the social services team and other staff
- Stay up-to-date on regulatory changes and professional standards related to long-term care social work
- Advocate for residents and serve as a liaison among staff, families, and external agencies
- Collaborate with interdisciplinary care teams to ensure holistic, person-centered care
- Duties as assigned
- Proficient Knowledge of Point click Care
Requirements:
- Minimum of 3 years Director of Social Services experience in a Skilled Nursing or Long-term care facility
- Bachelor’s degree in Social Work (BSW), or in a human services field such as sociology, psychology, special education, or rehabilitation counseling, preferred
- Knowledge of federal and state long-term care regulations
- Strong interpersonal, communication, and problem-solving skills
- A resident-first mindset with a deep commitment to dignity, empathy, and respect
- Ability to manage sensitive situations with professionalism and compassion
- Must meet all applicable licensure and certification requirements in the state of practice

Pay: $70,000.00 - $75,000.00 per year
