We are seeking a mission-driven Primary Care Physician to join a growing outpatient clinic in the Bronx serving underserved populations through an integrated, whole-person care model.
This is an exciting opportunity for a physician who wants to do meaningful work while helping shape clinical workflows, care delivery, and patient outcomes within a developing community health program. While not a leadership role today, there is long-term potential for growth into Medical Director-level influence as the clinic expands.
Responsibilities

Provide comprehensive outpatient primary care services

Deliver care to underserved and high-need patient populations

Collaborate with behavioral health, substance use, and care management teams

Participate in team-based care coordination and interdisciplinary discussions

Support quality improvement initiatives and patient outcome goals

Assist with refining clinical workflows and operational processes

Ensure compliance with clinical standards and best practices

Build strong therapeutic relationships with diverse patient populations
Qualifications

MD or DO required

Active New York medical license or ability to obtain

Board Certified or Board Eligible in Family Medicine or Internal Medicine

Experience working with underserved or vulnerable populations preferred

Community health, FQHC, or integrated care experience highly valued
