Job Summary
We are seeking a compassionate and dedicated Direct Support Professional to join our team. In this role, you will provide essential support to individuals with developmental disabilities, assisting them in daily living activities and promoting their independence and well-being. Your responsibilities will include patient care, monitoring vital signs, and ensuring a safe and nurturing environment.
Key Responsibilities:
Skill Development:** Assist beneficiaries in learning new skills and improving existing skills in the following areas:
Interpersonal skills
Independent living
Community engagement
Self-care
Self-determination
Individualized Support:** Provide tailored assistance based on each beneficiary’s unique preferences and needs to promote self-sufficiency and active community participation.
Supervision and Assistance:** Offer necessary supervision and support for beneficiaries to complete activities aligned with their level of independence. Areas of focus include:
Monitoring health conditions
Nutrition and physical well-being
Daily living skills
Community participation
Interpersonal relationship skills
Collaboration with Unpaid Supports:** Work alongside family members or natural supports who live with the beneficiary, providing technical assistance to help maintain and reinforce skills learned. This collaboration should be a joint decision between the paraprofessional, the unpaid supports, and the Individual Support Planning team.
Documentation and Reporting:** Maintain accurate records of support activities, progress toward goals, and any adjustments needed in the support plan. Ensure compliance with all relevant policies and procedures.
Community Engagement:** Encourage and facilitate opportunities for beneficiaries to participate in community activities, fostering social connections and enhancing their presence within the community.
Experience
Ideal candidates will have experience in the following areas:
- Working with people with developmental disabilities, autism, or dementia care.
- Group home or assisted living environments, including nursing homes or hospice care settings.
- Home & community care experience that includes caregiving responsibilities.
- Skills in first aid and CPR certification are preferred.
- Familiarity with patient monitoring techniques and vital sign assessment is beneficial.
