Telehealth Nurse Practitioner (Per Diem / Independent Contractor)

Health Connect Medical Group (HCMG) is seeking compassionate, technology-driven, and patient-focused Nurse Practitioners to provide high-quality telehealth services in collaboration with supervising physicians. Providers will support patients across multiple specialties including Primary Care, Neurology, Podiatry, and Wound Management through virtual visits conducted in patients' homes with assistance from Medical Assistants.
This role is ideal for providers who enjoy delivering patient-centered care while utilizing innovative telehealth technologies to improve access and outcomes.
No prior experience in Neurology, Podiatry, or Wound Care is required. Comprehensive specialty-specific training, clinical protocols, and ongoing physician support will be provided.

Essential ResponsibilitiesPrimary Care
- Perform telehealth evaluations for acute and chronic conditions.
- Review medical history, medications, laboratory results, and diagnostic studies.
- Manage hypertension, diabetes, COPD, CHF, hyperlipidemia, and other chronic illnesses.
- Develop treatment plans and provide patient education.
- Order laboratory studies, imaging, and referrals when appropriate.
- Prescribe medications according to state regulations and collaborative agreements.
Neurology
- Conduct neurological telehealth follow-up visits.
- Evaluate headaches, neuropathy, Parkinson's disease, dementia, epilepsy, stroke follow-up, and movement disorders.
- Monitor treatment effectiveness.
- Coordinate care with supervising neurologists.
- Review diagnostic imaging and neurological testing.
Podiatry
- Evaluate common foot and ankle conditions via telehealth.
- Assess diabetic foot concerns.
- Monitor healing after podiatric procedures.
- Coordinate referrals for in-office treatment when necessary.
- Educate patients regarding diabetic foot care and prevention.
Wound Management
- Evaluate chronic wounds through telehealth.
- Review wound photographs and documentation provided by Medical Assistants.
- Develop wound treatment plans.
- Recommend dressing changes and advanced wound care protocols.
- Monitor healing progress.
- Coordinate escalation of care when appropriate.
General Clinical Responsibilities
- Perform comprehensive telehealth assessments.
- Document encounters accurately within the Electronic Health Record (EHR).
- Utilize clinical decision-making based on evidence-based practice.
- Collaborate with physicians and interdisciplinary care teams.
- Participate in quality improvement initiatives.
- Maintain compliance with HIPAA and all applicable healthcare regulations.
- Support Remote Patient Monitoring (RPM), Chronic Care Management (CCM), and other value-based care initiatives when applicable.
Telehealth WorkflowProviders will conduct telehealth visits while Medical Assistants are physically present in the patient's home.Medical Assistants will assist with:
- Patient intake
- Vital signs
- Medication reconciliation
- Medical history review
- Point-of-care testing (when applicable)
- Camera positioning
- Technology setup
- Documentation support
- Care coordination
Providers focus primarily on clinical evaluation, diagnosis, treatment planning, and medical decision-making.Documentation Responsibilities
- Complete encounter documentation promptly.
- Maintain accurate ICD-10 coding.
- Maintain appropriate CPT coding when applicable.
- Complete electronic prescriptions.
- Document medical necessity.
- Complete referrals and follow-up recommendations.
- Sign all notes within organizational timelines.
QualificationsRequired
- Active California Nurse Practitioner License
- Active DEA Registration (when prescribing controlled substances)
- Active National Provider Identifier (NPI)
- Current BLS Certification
- Excellent communication skills
- Strong computer skills
- Ability to work independently in a telehealth environment
Preferred
- Experience in Primary Care
- Experience with telemedicine
- Experience using Electronic Health Records (Office Ally preferred but not required)
- Bilingual (English/Spanish or Tagalog) preferred

No prior specialty experience is required. Providers will receive ongoing mentorship, clinical support, and continuing education from supervising physicians and experienced clinical leadership.
