Job Overview
We are actively seeking full-time and part-time Licensed Practical Nurses (LPNs) to join our dedicated team. In this vital role, you will deliver high-quality nursing care to individuals with diverse needs. Your expertise will help promote health, safety, and well-being for our clients across various care environments.
LPN Duties
- Provide comprehensive patient assessments, vital signs monitoring, and medication administration in accordance with healthcare protocols.
- Assist with medical documentation using electronic health records (EHR) systems such as Epic or Athenahealth to ensure accurate record-keeping.
- Support patients with complex needs including IV infusion, catheter care, tube feeding, and ventilator management.
- Collaborate with interdisciplinary teams on discharge planning, case management, and individualized care plans tailored to each client’s requirements.
- Deliver specialized care for individuals with dementia, Alzheimer's disease, or memory impairments within memory or hospice care settings.
- Conduct physical examinations and diagnostic evaluations while adhering to infection control policies and aseptic techniques.
- Assist in emergency situations utilizing airway management skills and basic life support certification to ensure patient safety.
- Maintain compliance with long-term care regulations and HIPAA confidentiality standards while providing compassionate patient-centered care.
LPN Experience
- Prior experience working as an LPN in medical-surgical units, ICU, emergency room, or trauma centers is highly desirable.
- Demonstrated proficiency with EMR/EHR systems such as Cerner or eClinicalWorks for documentation and patient tracking.
- Experience caring for geriatric patients in skilled nursing facilities or assisted living environments is preferred.
- Knowledge of ICD coding (ICD-9/10), medication administration protocols, and diagnostic procedures enhances your effectiveness in this role.
- Background in working with individuals with developmental disabilities or behavioral health needs is a plus.
- Critical care experience including ICU or Level I trauma center exposure will be considered an asset.
- Strong understanding of physiology, anatomy, pharmacology, and pain management principles supports optimal patient outcomes.
