Licensed Practical Nurse (LPN) Job Summary:
Springvale Nursing and Rehabilitation Center, a leading senior care provider in Croton-on-Hudson, NY, is seeking a dynamic and compassionate Licensed Practical Nurse (LPN) to join our team. Licensed Practical Nurse (LPN) Responsibilities:
Provide direct patient care to residents, including administering medications, performing wound care, and assisting with daily living activities
Monitor and report changes in resident condition to the nursing team and medical staff
Collaborate with interdisciplinary team members to develop and implement individualized care plans
Maintain accurate and up-to-date resident records and reports
Develop and maintain positive relationships with residents, families, and colleagues
Participate in quality improvement initiatives and maintain a safe and clean environment

Licensed Practical Nurse (LPN) Requirements:
Current LPN licensure in the State of New York
Prior experience preferred (new graduates welcome to apply!)
Strong communication and interpersonal skills
Must be available to work alternate weekends
