Duties
- Assess patients’ physical conditions by conducting comprehensive evaluations of mobility, strength, and functional abilities
- Develop and implement tailored care plans that align with patients’ goals and medical needs
- Perform therapeutic interventions such as trigger point therapy, orthopedics techniques, and geriatrics-focused exercises to promote recovery
- Manage discharge planning processes to ensure seamless transition from hospital or clinic to home or community settings
- Document patient progress accurately using electronic medical record (EMR) systems and communicate updates with healthcare teams
- Provide specialized care for pediatric, geriatric, inpatient, outpatient, and home health populations
- Educate patients and caregivers on physiotherapy exercises, injury prevention strategies, and health maintenance practices
Requirements
- Valid licensure as a licensed Physical Therapist (PT) in the relevant jurisdiction
- Proven experience in acute care, hospital environments, outpatient clinics, or home & community care settings
- Strong knowledge of physiology, anatomy, medical terminology, and orthopedic principles
- Demonstrated ability to develop effective care plans for diverse patient populations including pediatrics and geriatrics
- Familiarity with EMR systems for documentation and patient management
- Excellent patient care skills with a compassionate approach to treatment and education
- Ability to work independently and collaboratively within multidisciplinary teams
