Requirements
A minimum of two years of work experience as an Emergency Housing Case Manager is required and experience using the HMIS system and similar data systems such as CaseWorthy, FYI, HUD Exchange, etc.
Must have experience in Public Housing Sector; Permanent Supportive Housing (i.e. Emergency Housing, Rapid Re-Housing, and ReEntry Housing Program)
Emergency Housing Case Manager (Male Residential Shelter)
The Emergency Housing Case Manager works directly with male clients in a residential shelter setting experiencing homelessness in quickly obtaining and maintaining stable housing. This role focuses on assessing client needs, developing individualized housing and care plans, and connecting clients to resources such as mental health services, job training programs, healthcare, helping them navigate the process of finding suitable housing options, accessing financial assistance needs, and addressing any barriers they may face in securing and maintaining housing stability. The Emergency Housing Case Manager will work collaboratively with shelter staff, community partners, and service providers to ensure clients receive comprehensive support.
Client Assessment and Case Management:
- Conduct intake assessments to identify clients' immediate needs, risks, and goals.
- Develop and implement personalized case management plans that address barriers to housing and self-sufficiency.
- Provide crisis intervention, counseling, and support to clients in addressing mental health, substance abuse, and personal challenges.
Case Planning:
- Developing individualized case plans in collaboration with clients, setting goals for obtaining and maintaining housing stability, and outlining steps to achieve those goals.
Housing Placement and Support:
- Work with clients to find and secure safe, stable housing options.
- Assist clients in understanding lease agreements, tenant rights, and responsibilities.
- Help clients access rental assistance programs and support them in navigating housing applications.
- Assisting clients in identifying available housing options that meet their needs and preferences, including affordable rentals, transitional housing, or other supportive housing programs.
Resource Coordination:
- Connect clients with local services such as mental health providers, addiction recovery programs, healthcare services, and job training opportunities.
- Coordinate with local agencies to secure housing vouchers, legal aid, and other resources for clients.
Case Documentation and Reporting:
- Maintain accurate and confidential case notes and records for each client in compliance with organizational policies and funder requirements.
- Report on client progress, housing placements, and outcomes for program evaluation.
Skill Development and Advocacy:
- Facilitate life skills workshops and one-on-one sessions covering topics like budgeting, job search techniques, communication, and conflict resolution.
- Advocate for clients' rights and needs within the shelter and with external service providers.
Follow-Up and Aftercare:
- Conducting follow-up assessments and providing aftercare support to clients after they have secured stable housing, to ensure they maintain housing stability and address any emerging needs or challenges.
Qualifications and Competencies
Must have a minimum of two years of work experience as an Emergency Housing Case Manager is required and experience in Public Housing Sector; Emergency Housing, Permanent Supportive Housing (i.e. Rapid Re-Housing, ReEntry Housing Program)
Proficient in Emergency Housing Case Management
- Strong interpersonal and communication skills.
- Ability to work with a diverse population and address sensitive issues compassionately.
- Familiarity with housing resources, community services, and the challenges faced by individuals experiencing homelessness.
- Excellent organizational skills and proficiency in case management software.
- Ability to work flexible hours, including evenings and weekends if needed.
- Commitment to a trauma-informed, client-centered approach.
- Valid driver's license and reliable transportation (if required).
Time management
- Excels at time management and organization.
- Demonstrates competence in task prioritization and resourcefulness.
- Capable of balancing multiple demands concurrently.
- Supports staff with time efficiency to meet tight deadlines.
Technical skills
- Proficient with Microsoft Word, Outlook, and PowerPoint; has knowledge of spreadsheets and advanced Excel skills.
- Experienced in using Zoom, Teams, Skype, and other virtual meeting platforms.
- Willing to learn and use new software and databases as needed, including Homeless Management Information System (HMIS).
