Academic: Minimum – Bachelor’s Degree in Social Work, Required License- LSW or LSW-S
Experience: Social Work/Foster Care: 1 year
JOB PURPOSE
The Licensed Social Worker works to help with creating a treatment plan and developing a service plan for each resident. An LSW or LSW-S will be assessable 24 hours a day, seven days a week.
DUTIES AND RESPONSIBILITIES
1. Actively support, represent, and extend the mission of APOH through direct services, customer contact, and all other assigned functions.
2. Dedicate a minimum of 50% of the time in direct service of youth and families
3. Support appropriate placement and/or services as needed.
4. Complete all required documents on time as per agency protocol.
5. Develop and regularly review youth service plans with youth, referring agency, caseworker, authorized family members, and the team at APOH.
6. Maintain youth contact including face-to-face interaction per state and local requirements.
7. Assist in permanent living arrangements consistent with the youth’s case plan.
8. Follow agency procedures for after-hours and weekend emergency coverage.
9. Respond directly to emergencies and crises with youth as deemed appropriate or as necessary to ensure safety and stability.
10. Serve as a liaison between referring agency caseworkers and probation officers regarding the youth’s services.
11. Participate in the Quality Assurance activities of APOH.
