Always Best Care Home Care- Cleveland South is searching for motivated and experienced nurses to join our team. We provide skilled nursing and therapy services to the community and facility-based patients. This is a great opportunity that allows flexibility. The first territory includes Parma and the surrounding suburbs. The second territory includes the Garfield Heights area and the surrounding suburbs. The third territory is the surrounding cities of Beachwood. There is also availability in Cleveland (westside).
This position can be full-time or part-time between the hours of 8a-4:30p. PRN is not available.

Qualifications:
- Current unrestricted license in the state of Ohio
- BLS certification
- Minimum two year's experience as a licensed nurse
- Valid driver’s license and reliable transportation
- Excellent customer service and communication skills
- The ability to work independently with an emphasis on time management
- Computer skills
- Knowledgeable and competent in wound care, foley catheter management, disease process management, and IV administration
- Attention to detail
Job Responsibilities:
- Provides direct skilled nursing care per Always Best Care policy.
- Completes thorough assessments and reports to the physician any changes in status to decrease the patient’s risk of re-hospitalization.
- Maintains open communication with all disciplines involved in the patient's care.
- Completes documentation timely in the electronic medical record.
