Job Summary
We are seeking a dedicated and compassionate Chronic Care Management Nurse to join our dynamic healthcare team. Duties
- Conduct thorough patient assessments, including vital signs, medical history review, and physiological evaluations to determine care needs.
- Develop and implement individualized care plans in collaboration with physicians, specialists, and other healthcare providers.
- Manage post-acute care transitions by coordinating discharge planning and ensuring continuity of care across inpatient, outpatient, and community settings.
- Monitor patient progress through regular follow-ups via phone calls, telehealth visits, or in-person appointments to address concerns and adjust treatment plans accordingly.
- Educate patients and their families on disease management strategies, medication adherence, lifestyle modifications, and available community resources.
- Utilize medical records and diagnostic evaluation data to inform case management decisions and optimize utilization management processes.
- Maintain strict compliance with HIPAA regulations and ensure accurate documentation of all patient interactions in medical records systems.
Skills
- Proven experience in critical care environments such as ICU or emergency medicine settings with a strong understanding of hospital medicine protocols.
- Experience working within Level I or Level II trauma centers, including trauma assessment and management skills.
- Knowledge of infectious disease care, geriatrics, behavioral health, pediatrics (PICU), and post-acute care practices.
- Strong background in medical-surgical nursing, managed care principles, and outpatient clinic operations.
- Excellent patient service skills with the ability to perform comprehensive patient assessments and vital signs monitoring.
- Familiarity with medical terminology, diagnostic evaluation procedures, discharge planning processes, and medical records documentation.
- Ability to coordinate case management activities effectively across inpatient and outpatient settings while adhering to HIPAA standards.
- Skilled in utilization management strategies to optimize resource use without compromising patient safety or quality of care.
