Job Overview
A Community Health Worker (CHW) is a trusted individual who contributes to improved health outcomes in the community and has a close understanding of the community served. The CHW serves as a liaison between health and social services and individuals in the community to facilitate access to services. CHWs serve the communities in which they reside and/or communities with which they share ethnicity, language, socioeconomic status, or life experiences.
Job Responsibilities
- Conducts outreach to clinics, community centers, emergency departments and other community sites, to find individuals from the target population eligible for enrollment into the Brazos Healthy Communities HUB.
- Verifies eligibility and completes all documentation and tools as needed for enrolling the population being served.
- Develops individualized plans addressing maternal / child health, chronic disease, housing, food, and employment.
- Works with enrolled participants in the community and their homes to identify needs and connects them to community-based resources and services to remove barriers to care.
- Completes intervention plan and case related documentation within the time frame of the established policy.
- Ensures participants schedule and attend medical appointments with any providers, and assists with transportation to these appointments as needed.
- Educates enrolled participants on health topics relevant to their situation using the PCHI® Model Learning Modules.
- Maintains a full-time active caseload of no greater than 30 low-risk participants and no greater than 20 high-risk participants.
- Utilizes Care Coordination System (CCS) software to track and monitor participant progress, as well as Intake1, Project Unity’s client management system, to monitor household-level progress and outcomes.
- Attends Brazos Healthy Communities HUB and PCHI required trainings and monthly meetings, and attends 2-3 community events/health fairs annually promoting the agency and Brazos Healthy Communities HUB.
- Obtains CHW certification (where possible) within one year of hire.
- Performs other duties as assigned
Qualifications
- Minimum of High School Diploma or GED equivalent required
- Have or have the ability to obtain a Community Health Worker certification
- One (1) to three (3) years of recent related experience
- Strong interpersonal and communication skills
- Bilingual in English / Spanish is a plus
- General knowledge of medical terminology
- Ability to communicate effectively, function independently, manage time efficiently, organize workflow, and maintain confidentiality
- General knowledge of community resources
- Strong analytical and problem-solving skills
- Strong ability in understanding documentation requirements
Pay: $20.60 per hour
