Pay Rate: $ 2,500 To $ 2,720 Per Week
Contract: 12 Weeks
Schedule: Day Shift | Monday Friday
Experience: Minimum 1 Year of Recent Clinical Nursing Experience Preferred
About the Role
We are seeking a compassionate Home Health & Hospice Registered Nurse (RN) for a 12-week contract opportunity in St. Johnsbury, Vermont. The Registered Nurse will provide skilled nursing care to patients in home health and hospice settings by performing assessments, coordinating individualized care plans, and collaborating with an interdisciplinary healthcare team to deliver high-quality, patient-centered care.
Requirements
Active Vermont Registered Nurse (RN) License required
Minimum 1 year of recent clinical nursing experience in Home Health, Hospice, or a related setting preferred
Current BLS Certification required
Experience with Homecare Homebase (HCHB) EMR preferred
Strong clinical assessment, care coordination, and patient education skills
Excellent communication and documentation skills with the ability to work independently
