Position: Part-Time Registered Nurse Case Manager
Department: Guardian Health
Reports To: Director of Nursing
Status: Part-time
Hours: Part-Time; schedule based on client and agency needs
Position Summary
The Part-Time Registered Nurse Case Manager reports directly to the Director of Nursing and plays a critical role in ensuring clients receive safe, professional, and compassionate care in their homes. ThePart-time Registered Nurse Case Manager will complete in-home clinical assessments, provide supervision of aides, and provide guidance and training to caregivers to ensure the highest quality of care for our patients. The RN Case Manager serves as a key member of the clinical team, helping maintain high standards of care, improve patient outcomes, and support continuity of care across the organization.
Responsibilities/Job:
Clinical Assessment & Care Management
- Conduct in-person patient interviews and comprehensive physical assessments.
- Develop, implement, and monitor individualized plans of care.
- Evaluate patient conditions and adjust care plans as needed.
- Collaborate with physicians, providers, and interdisciplinary team members to coordinate care.
- Provide direct patient visits and clinical support.
- Promote preventive care initiatives to reduce hospital readmissions and improve outcomes.
Staff Supervision & Education
- Supervise Home Health Aides, LPNs and other caregivers to ensure quality care delivery.
- Provide clinical guidance, training, and support to caregivers.
- Educate patients and family members on home health care procedures, including wound care, medication management, IV administration, and disease management.
- Ensure caregivers follow established care plans.
Documentation, Compliance & Quality Assurance
- Maintain accurate and timely patient documentation and medical records.
- Participate in QAPI activities as assigned.
- Ensure adherence to OASIS guidelines and standards.
- Support compliance with Medicare, ACHC, and other applicable regulatory requirements.
- Maintain HIPAA compliance and confidentiality of Protected Health Information (PHI).

Required Qualifications:
- Current, unrestricted Indiana Registered Nurse (RN) license.
- Current health care provider CPR.
- Minimum of one (1) year of professional nursing experience in home health preferred.
- Extensive OASIS experience and demonstrated knowledge of OASIS compliance requirements.
- Strong clinical assessment and physical examination skills.
- Knowledge of medical terminology, pharmacology, and care planning.
- Ability to interpret medical test results and develop appropriate plans of care.
- Excellent communication, critical thinking, and problem-solving skills.
- Active driver's license and reliable transportation.
- Ability to travel regularly throughout assigned service areas.
- Fine motor skills, visual acuity, and auditory perception necessary for patient assessments and documentation.
- Must be authorized to work in the United States.
Preferred Qualifications:
- Medicare experience.
- ACHC and/or CHAP accreditation experience.
- Additional home health leadership or supervisory experience.
- Experience working with interdisciplinary healthcare teams.
- Familiarity with electronic health records and home health software platforms.
Work Environment:
- Primarily community-based with frequent travel to patient homes.
- Combination of field work and remote administrative responsibilities.
- Ability to drive regularly and make home visits throughout assigned territory.
- Frequent standing, walking, bending, squatting, kneeling, reaching, and stair climbing.
- Ability to lift up to 50 pounds and occasionally assist with patient transfers.
- Exposure to bloodborne pathogens and infectious materials is expected; appropriate PPE training and equipment will be provided.
- Occupational Exposure Risk: Category I – Routine Exposure.
- Must be able to tolerate varying home environments, including temperature changes, allergens, pets, and other environmental conditions.
