Minimum Requirements
Graduate of an accredited practical nurse or vocational nursing program.
One year nursing experience, or at least six months experience if concurrently working in a Hospital or Home Health Setting.
Community health/hospice or medical/surgical experience is preferred.
Currently licensed as an LPN/LVN in the state of California.
Complies with accepted professional standards and practice.
Demonstrates good verbal and written communication skills.
Demonstrates good organization skills.
Must be a licensed driver with an automobile that is insured in accordance with state and/or organization requirements and is in good working order.
Understands hospice philosophy. Also understands needs of the terminally ill.
Essential Job Functions and Responsibilities

Provide direct patient care as defined in the State Nurse Practice Act.

Implement current nursing practice following a patient assessment and an approved Plan of Care initiated by the Registered Nurse.

Provide accurate and timely documentation of patient services in accordance with the Plan of Care.

Assess and provide patient/family/caregiver education and information pertinent to the patient’s diagnosis and Plan of Care.

Participate in the coordination of hospice services, appropriately reporting the identified needs to hospice Interdisciplinary Group members. Examples include hospice aide, OT, PT, MSW, ST, Dietitian, and Clinical Supervisor.

Use equipment and supplies effectively and efficiently.

Provide appropriate pain/symptom management. Evaluate the patient’s response to treatments/medications.

Participate in personal, professional growth and development, as well as in the organization's quality management program.

Perform other duties as assigned by the RN Case Manager.
