Responsibilities
- Assist clients with activities of daily living (ADLs), including bathing, dressing, grooming, and mobility support.
- Monitor patients’ health status by observing vital signs, medication adherence, and overall condition; report any changes to healthcare professionals promptly.
- Prepare nutritious meals tailored to individual dietary requirements and assist with feeding as needed.
- Support patients with mobility using proper patient transfer methods such as Hoyer lifts or manual handling techniques to ensure safety.
- Maintain accurate care plans and document all care activities, observations, and incidents in compliance with HIPAA regulations.
- Provide companionship and emotional support to foster a positive environment for clients with dementia, Alzheimer’s, or other neurodevelopmental disorders.
- Assist with medication administration following prescribed schedules and safety protocols; ensure proper storage and documentation.
- Perform light housekeeping tasks such as cleaning, laundry, and maintaining a safe living environment for clients.
- Support clients during community outings or personal transportation needs to promote social engagement and independence.
- Follow all home health standards and protocols related to hospice care, memory care, or long-term senior care to ensure high-quality service delivery.
Requirements
- Proven experience working as an In Home Caregiver or in related settings such as group homes, assisted living facilities, nursing homes, or home health environments.
- Strong knowledge of patient observation techniques, vital signs monitoring, medication administration, and first aid/CPR certification.
- Familiarity with specialized care for dementia, Alzheimer’s disease, autism spectrum disorder (ASD), intellectual disabilities, and developmental disabilities support.
- Ability to perform manual handling tasks including heavy lifting and patient transfers using Hoyer lifts or other equipment safely.
- Excellent communication skills to effectively interact with clients, families, and healthcare teams while maintaining confidentiality per HIPAA standards.
- Capable of assisting with personal transportation and community outings while ensuring client safety at all times.
- Flexibility to adapt to different client needs across various settings such as geriatrics or hospice environments.
- Valid driver’s license preferred for client transportation assistance; reliable vehicle may be required depending on the role.
