Hospice of South Texas is seeking a compassionate and skilled Licensed Vocational Nurse (LVN) to provide direct patient care in a home-based setting. Working under the supervision of a Registered Nurse, the LVN plays a critical role in supporting patients and families through end-of-life care while collaborating closely with the interdisciplinary team.

Deliver skilled nursing care in accordance with the plan of care (POC) under RN supervision

Support the RN in implementing and coordinating patient care needs

Assist patients and families with education on self-care and disease management

Document all clinical care, interventions, and patient responses accurately and timely

Communicate changes in patient condition to the RN and care team

Assist with procedures and specialized care related to end-of-life support

Prepare and maintain equipment and supplies using proper aseptic techniques

Ensure care is delivered in compliance with organizational policies and regulatory standards

Promote the mission, values, and quality standards of Hospice of South Texas

Participate in interdisciplinary collaboration to ensure continuity of care

Perform other duties as assigned
EducationLicensure
-
Graduate of an accredited vocational nursing program
-
Current Licensed Vocational Nurse (LVN) license in the State of Texas
-
Valid Texas Driver's License
Experience
-
Minimum of 2 years of clinical nursing experience

Hospice or home health experience preferred

Strong clinical nursing skills aligned with accepted standards of practice

Excellent communication and interpersonal skills

Ability to respond effectively in emotionally sensitive situations

Strong documentation and organizational skills
Other Requirements

Reliable transportation and valid auto liability insurance

Proof of Hepatitis B vaccination or declination

Provide care in patients' homes with varying conditions

Frequent standing, walking, bending, and lifting (up to 50 lbs.)

Exposure to bloodborne pathogens and infectious diseases
-
Ability to work flexible schedules, including on-call or PRN emergency situations

Local travel required
