Wayne Memorial Community Health Centers creates and sustains healthy rural communities by providing accessible, high quality medical, dental, and behavioral health care for all, while recognizing and addressing other factors that influence health outcomes.
Obtains complete medical history and physical data on patients.
Interprets and integrates data to determine appropriate diagnostic and therapeutic procedures as needed.
Performs a complete physical exam and records findings. Collaborates with physicians in managing acute and long-term medical needs of patients.
Orders appropriate laboratory and diagnostic procedures.
Synthesizes data to determine preliminary diagnosis and therapeutic plan utilizing principles of prevention.
Administers medications and injections according to physician direction. Sutures minor lacerations.
Interviews and advises patients regarding health and illness prevention. Recommends community resources to meet patient and family needs.
Instructs patient and family regarding medications and treatment instructions. Provides patient education.
Maintains and reviews patient records, charts, and other pertinent information. Posts tests and examination results.
Triages patient telephone calls and provides consultation.
Manages medical and surgical emergencies.
Provides monitoring and continuity of care between physician visits.
Provides comprehensive health care to pediatric and adolescent patients as indicated. This may include assessment of the newborn, well child care, adolescent health care, management of acute and chronic diseases and treatment of injuries.
Per State Licensing guidelines, prescribe a limited formulary of medication.
Provide workshops, seminars and presentations to other professionals and community.
Develop and evaluate relevant patient education tools as appropriate.
As appropriate, act as director or participant of special community programs and projects.
Performs other duties as assigned to support efficient operation of the department.
Graduate of an accredited program
Knowledge of professional nursing theory, practice, and medical care to give and evaluate patient care
Knowledge of organizational policies, regulations and procedures to administer patient care
Knowledge of medical equipment and instruments to administer patient care;
Knowledge of common safety hazards and precautions to establish a safe work environment
Skill in applying and modifying the principles, methods, and techniques as a professional health care provider; in taking medical history to assess medical condition and interpret findings
Skill in preparing and maintaining records, writing reports, and responding to correspondence
Skill in developing and maintaining department quality assurance, in establishing and maintaining effective working relationships with patients, medical staff, and the public
Ability to maintain quality control standards and ability to react calmly and effectively in emergency situations
Ability to interpret, adapt and apply guidelines and procedures and to communicate clearly
