Licensed Practical Nurse (LPN) – Mobile Wound Care

About the Role
Advanced Comfort Health is seeking a motivated Licensed Practical Nurse (LPN) to join our mobile wound care team, delivering UltraMist therapy to patients throughout the Twin Cities and surrounding areas. This is a unique opportunity to provide specialized wound care services directly at patient locations, working with a growing healthcare organization dedicated to improving outcomes for patients in Skilled Nursing and Assisted Living communities.
Pay: $36.00 per hour
Location: Minneapolis, MN (mobile/field-based role — travel required throughout the Twin Cities metro and surrounding areas)
Responsibilities
- Administer UltraMist ultrasonic wound therapy to patients per treatment protocols and provider orders
- Travel to Skilled Nursing Facilities (SNFs), Assisted Living Facilities (ALFs), and other patient locations across the Twin Cities and surrounding areas
- Perform wound assessments and document care using eKare wound imaging/documentation technology
- Maintain accurate, timely clinical documentation in compliance with company and regulatory standards
- Communicate effectively with Nurse Practitioners, facility staff, and care teams regarding patient status and treatment plans
- Ensure proper handling, transport, and maintenance of UltraMist equipment and supplies
- Follow infection control protocols and maintain a high standard of patient care during all visits
- Manage a daily travel route/schedule efficiently to meet patient visit windows
Qualifications
- Active, unencumbered LPN license in the State of Minnesota
- Reliable transportation and willingness to travel daily throughout the Twin Cities and surrounding areas
- Prior wound care experience preferred; UltraMist or wound therapy experience a plus
- Comfortable working independently in a mobile/field-based role
- Strong documentation and communication skills
- Experience with EHR/wound documentation systems (eKare, MatrixCare) a plus
- Valid driver's license and clean driving record
