Care Corps' LifeHouse is seeking a compassionate, organized, and motivated Shelter Case Manager to join our team. In this role, you'll work directly with individuals experiencing homelessness, helping them create personalized plans that lead to stable housing, employment, improved health, and greater independence. What You'll Do
- Build meaningful relationships with shelter residents through weekly one-on-one case management.
- Develop individualized service plans and help participants achieve their goals.
- Connect clients with housing, employment, education, healthcare, mental health, substance use treatment, and community resources.
- Teach monthly life skills classes that empower participants to build confidence and independence.
- Collaborate with schools, landlords, employers, and community agencies to coordinate services.
- Maintain accurate documentation.
- Monitor participant outcomes and celebrate progress toward permanent housing and self-sufficiency.
- Work alongside an experienced team committed to serving our community with dignity and respect.
Preferred Qualifications
- Associate's degree or higher in Human Services or a related field.
- Experience in case management, human services, HMIS, or direct client services.
- Bilingual applicants are encouraged to apply.
- Valid driver's license, reliable transportation, and required auto insurance.
Pay: $20.00 - $22.00 per hour
