Position Summary:
Fairfield Community Health Center, a Federally Qualified Health Center (FQHC) is seeking a School Based Physician Assistant OR Certified Nurse Practitioner. This is a salaried and 1.0 FTE position working under the supervision of the Chief Medical Officer with a signed Standard of Care Arrangement for Advanced Practice Nursing or Supervisory Agreement for Physician Assistants. The employee is assigned to a collaborating physician in accordance with Ohio Board of Nursing and Ohio Medical Board meeting requirements as applicable and works closely with physicians, school personnel, and other clinicians in a team-based approach to patient care.
This position is primarily based within a school-based health clinic located in partnership with Lancaster City Schools, providing care to school-aged children and adolescents, while also serving members of the surrounding community.
This position is responsible for providing comprehensive primary care services, including assessment, diagnosis, treatment, prescribing, and patient education. Based on patient volume and operational needs, the provider may also deliver care at the organization’s brick-and-mortar clinical sites.
Responsibilities and Duties:
- Promotes a healthy lifestyle including smoke-free living, stress reduction, exercise, and balanced nutrition, with an emphasis on pediatric and adolescent wellness.
- Participates in and promotes health and wellness initiatives within both the clinic and school-based setting.
- Supports a Patient-Centered Medical Home (PCMH) model, emphasizing continuity of care, care coordination, and long-term patient relationships.
- Facilitates timely access to care within the school environment, minimizing disruption to student learning.
- Collaborates with school staff (nurses, counselors, administrators) to support student health and well-being.
- Serves as a member of the health care team providing quality care to students, families, and community patients.
- Maintains effective working relationships with supervising physician and care team members.
- Coordinates patient services and referrals to ensure comprehensive, continuous care across school and clinic settings.
- Incorporates preventive care practices tailored to age, developmental stage, and social determinants of health.
- Provides education to patients and families to support informed decision-making and improved health outcomes.
- Advises and counsels patients and families on growth, development, and behavioral health concerns, particularly in pediatric and adolescent populations.
- Assesses and intervenes in common developmental, behavioral, and psychosocial concerns in collaboration with families and school personnel when appropriate.
- Collects and evaluates patient histories (medical, social, family) and performs physical exams across the lifespan, with emphasis on pediatric care.
- Performs preventive services including immunizations, screenings, and well-child visits.
- Diagnose and treats common acute and chronic conditions; consults or refers complex cases as appropriate.
- Provides patient education and follow-up instructions; engages family members when appropriate.
- Documents care in the Electronic Medical Record (EMR) accurately and in a timely manner.
- Supports management of stable chronic conditions and coordinates care for patients requiring higher levels of service.
- Provides reproductive and contraceptive counseling in accordance with regulations and organizational policy.
- Prescribes medications and treatments in accordance with Ohio law and supervision or standard of care agreement.
- Responds to urgent or emergent health needs within the clinic setting.
- Follows infection control and universal precautions.
- Uses supplies and resources efficiently and responsibly.
- Performs clinic and school-based outreach activities as needed.
- Participates in Quality Improvement (QI) and PCMH initiatives.
- Demonstrates strong communication and teamwork skills across clinical and school environments.
- Builds and maintains professional relationships with healthcare team members and school partners.
- Participates in care planning, workflow improvement, and problem-solving initiatives.
- Engages in continuing education and professional development.
- Attends staff meetings and required trainings.
- Maintains compliance with all organizational, state, and federal regulations (HIPAA, OSHA, etc.), including student privacy considerations where applicable.
- Supports seasonal and community-based programs as needed.
- Demonstrates flexibility and adaptability in response to changing patient volume, school schedules, and clinic needs, including working at alternate clinic sites as assigned.
- Performs other duties as assigned.
Qualifications (NP and PA):
- Graduate of an accredited Physician Assistant program (Master’s level preferred.)
- Master of Science in Nursing Required
- AANP or ANCC certification as a Nurse Practitioner required.
- Current license to practice as a Physician Assistant in the State of Ohio.
- Licensed as a Registered Nurse and Advanced Practice Registered Nurse in Ohio (ORC 4723)
- Current DEA license in good standing.
- BLS/CPR certification required.
- Experience in family practice, pediatrics, or community health strongly preferred.
- Ability to work effectively as part of a multidisciplinary team.
- Strong communication and interpersonal skills, particularly with children, adolescents, and families.
- Computer proficiency, including EMR systems, preferred.
- Ability to work with a culturally diverse population.
- Must have reliable transportation and willingness to travel between sites as needed.
- Must be willing to work flexible hours based on school and clinic needs.
Working Conditions/Environment:
Primary work location is a school-based clinic within Lancaster City Schools, with potential assignment to additional clinic locations based on patient care needs.
Work involves typical clinical activities including standing, walking, bending, and use of medical and office equipment. The role requires comfort working in a non-traditional clinical environment within a school setting, including interaction with children, adolescents, and school personnel.
Exposure to communicable diseases and clinical hazards is consistent with outpatient healthcare settings. Work schedules may vary based on school hours, clinic demand, and organizational needs.
