Job Summary

Responsibilities
- Assist clients with activities of daily living (ADLs), including personal hygiene, dressing, grooming, and mobility support using manual handling techniques such as Hoyer lifts and patient transfer methods
- Monitor client health by taking vital signs, observing patient conditions, and reporting changes promptly to healthcare teams or supervisors
- Support medication administration in accordance with care plans and HIPAA regulations to ensure safety and compliance
- Prepare nutritious meals and assist with meal planning to meet individual dietary needs while maintaining cleanliness in the kitchen area
- Provide transportation assistance for appointments, community outings, or errands using appropriate safety protocols
- Maintain a clean and safe environment by performing routine cleaning tasks in client living areas and adhering to infection control standards
- Document care activities accurately in client records, including progress notes, incident reports, and updates on care plans related to dementia care, Alzheimer’s care, or other specialized needs
Experience
- Prior experience working in group homes, assisted living facilities, nursing homes, or home health environments is highly desirable
- Demonstrated ability to provide patient care with patience and empathy while observing safety protocols such as first aid and CPR certification
- Knowledge of medication administration procedures and patient monitoring techniques
- Experience supporting individuals with autism, intellectual disabilities, developmental disabilities, or neurodevelopmental disorders is preferred
- Familiarity with care plans development and implementation for residents with dementia or memory care needs
- Ability to lift heavy objects safely and perform manual handling tasks required for patient transfers or heavy lifting activities
- Strong communication skills for effective client interaction and collaboration with healthcare teams
