EXAMPLE OF DUTIES
This position provides key support to individuals with developmental delays, mental health needs, or intellectual disabilities. The role focuses on connecting people with essential resources and tracking their progress.
As a County Caseworker 1, you will perform the following duties:

Service Monitoring: Responsible for monitoring, linking, and evaluating services. Link to other services and make referrals as needed

Direct Contact: Provide regular direct individual contact and provide face-to-face case management services while meeting productivity standards as set for the program

Plan Development: Schedule and facilitate meetings to develop an individualized service plan with involvement of the individual and/or family

Information Management: Enter necessary information and maintain information in computerized database, state computer systems, and in charts in accordance with state, federal and licensing regulations and Agency policy. Obtain & update information within required time frames

Crisis Support: Offer crisis intervention services and coordinate assistance as needed.

Collaboration: Work with Agency staff, providers, and community resources to support individuals

Transportation Support: Link individuals to transportation services or transport individuals to appointments as needed
In addition, you will attend meetings, staffings, and trainings as needed. You may provide coverage in Somerset County. You will perform other related duties as assigned.
Work Schedule and Additional Information:

Full-time employment.

Work hours are 8:00 AM - 4:00 PM, Monday - Friday, with a 30-minute lunch.

On occasion, staff may need to work hours outside of the set work week to complete projects, meet work demands or in the event of an emergency.

Hours may vary to meet the needs of the consumers.

This position reports to the DBHS Office in Bedford for work. At times, may provide coverage in Somerset County.

Telework: This position reports to the DBHS Office for work. Upon hire, new staff begin training and are on probation for 6 months. New staff may be eligible for a hybrid of on-site and remote work after 3-months of employment.
