Position Overview
A Caring Heart Home Services is Agency with a growing Adult Foster Living (AFL) and Personal Care Attendant (PCA) Program that promotes home care services to patients at the comfort of their homes.
Program Coordinator
Under the direct supervision of the Program Manager this position is responsible for overall Program coordination activities which include but not limited to:
- Understand the eligibility guidelines of Connecticut Department of Social Services programs: Connecticut Home Care Program for Elders that offer AFL and PCA Services.
- Understand All program details: Eligibility verification, referral, intake, admission and discharge processes.
- Client Admission
- HR Duties
- Triage Phone calls.
- Coordinate with the scheduler to schedule admissions within 48 hours of an approved referral
- Scan in documents to patient's charts
- Office administrative duties such as answering phones in a courteous manner
- Coordinates with the administrative team to complete audits to ensure Agency compliance with regulations
- Bilingual in English and fluent in Spanish is a required
