Skilled Nursing (RN/LPN)
Skilled Nursing Visits for clients in Rogers, St. Paul, and surrounding areas
Must have at least 1–2 days of consistent availability (4–8 hours per week). Flexible scheduling available.
Duties:
- Provide skilled nursing care in clients' homes in accordance with physician orders and individualized care plans.
- Perform comprehensive nursing assessments and monitor clients' health status.
- Administer medications, injections, and prescribed treatments.
- Provide wound care, catheter care, ostomy care, and other skilled nursing procedures.
- Monitor and document vital signs, symptoms, and changes in condition.
- Educate clients and family members on disease management, medications, and ongoing care needs.
- Coordinate care with physicians, therapists, caregivers, and other members of the healthcare team.
- Maintain accurate and timely clinical documentation in accordance with agency and regulatory standards.
- Respond appropriately to changes in client condition and communicate concerns to the appropriate healthcare providers.
Skills:
- Current RN or LPN license in the State of Minnesota (or applicable state license).
- Current CPR/BLS certification.
- Experience in home health, skilled nursing, long-term care, rehabilitation, hospice, or acute care preferred.
- Knowledge of nursing assessments, medication administration, and skilled clinical procedures.
- Strong clinical judgment and critical thinking skills.
- Excellent communication and interpersonal skills.
- Compassionate, dependable, and patient-focused approach to care.
- Ability to work independently while managing multiple priorities.
- Reliable transportation and a valid driver's license preferred.
