Position Summary
We are seeking a Licensed Clinical Social Worker (LCSW) or similarly qualified professional to serve as a Behavioral Health Care Manager. In this role, you will coordinate care, provide brief evidence-based interventions, and monitor patient progress in skilled nursing facilities. You will act as the key connection between patients, primary care providers, and our psychiatric consultant to ensure that patients receive the right care at the right time.
Key Responsibilities
- Conduct initial behavioral health assessments using validated tools (e.g., PHQ-9, GAD-7).
- Provide brief, evidence-based interventions such as behavioral activation, problem-solving therapy, and motivational interviewing.
- Maintain and update a patient registry to track clinical outcomes and ensure no patient is overlooked.
- Communicate patient progress weekly with the psychiatric consultant and collaborating provider.
- Facilitate warm handoffs from primary care providers to CoCM services.
- Engage patients in person, by phone, or via telehealth.
- Support systematic case reviews and implement treatment recommendations.
Qualifications
- Licensed Clinical Social Worker (LCSW) or equivalent (e.g., MSW with relevant training).
- Experience or training in integrated care models, brief interventions, and registry management preferred.
- Strong communication, organizational, and care coordination skills.
- Comfortable working in skilled nursing facilities and collaborating with a multidisciplinary team.
Schedule & Compensation
- Full-time, onsite role.
- Standard weekday schedule; flexibility may be required for patient needs.
- Salary rate of $85,000 - $100,000 per year, based on experience.
